Telemedicine Consent Form

Client and Patient Information

Definition of Telemedicine

Telemedicine involves delivering veterinary care using real-time audio-video communication technology, including phone calls and video conferencing. Under AB 1399, a veterinarian-client-patient relationship (VCPR) may be legally established via telemedicine through synchronous (real-time) audio-video communication, provided all required standards are met.

Veterinarian-Client-Patient Relationship (VCPR)

I understand that under California AB 1399, a valid VCPR may be established via synchronous audio-video telemedicine, without an initial in-person exam. Once established, the VCPR is valid for up to 6 months, unless an in-person exam extends it.

I also acknowledge that certain medications, including antimicrobials, may be prescribed for no more than 14 days following a telemedicine consultation unless an in-person exam is performed.

Technology and Limitations

I understand the potential risks and limitations of telemedicine, such as miscommunication, or limited ability to conduct physical exams. Despite these limitations, I consent to receive veterinary care for my pet via telemedicine when deemed appropriate by the veterinarian.

Emergency Situations

I understand that telemedicine is not appropriate for emergency situations. In case of a medical emergency, I will seek immediate in-person veterinary care.

Fees and Payment

I agree to pay a telemedicine consultation fee of $79 prior to the actual consultation. In cases where an in-person exam needs to be done for the same issue, 50% of the fee will count towards the in-person exam.
