Before scheduling an appointment for a health certificate, we ask you to fill out our questionnaire to provide us more information on your travel so that we can make the health certificate appointment smooth and efficient.

Please take into consideration that the cost of the international health certificate is $317.00 and an additional $38.00 to $120.00 (depending on the destination location) processing fee for electronic endorsement by the USDA. If you have any additional questions, please call us at 714-123-4567.
